In order to play with the Marion Soccer Club, the following forms must be completed: Player Registration Form Player Medical Release Form Player Code of Conduct Form Copy of State Birth Certificate (Proof of Age) If an entire team is moving to MSC, the Team Affiliation Form must be filled out. |
